Grand Oaks of Jensen Beach Earns Perfect AHCA Survey Score
Recently, Florida’s Agency for Health Care Administration (AHCA) conducted an unannounced re-licensure survey at Grand Oaks of Jensen Beach. Upon completion of the in-depth inspection, AHCA reported that Grand Oaks had zero deficiencies! The average number of deficiencies for a nursing or assisted living facility is seven, making Grand Oaks of Jensen Beach’s zero deficiencies all the more impressive.
The licensing and regulation of healthcare facilities throughout Florida is AHCA’s responsibility, as is providing the community with information about the quality of care provided by each healthcare facility. For these reasons, AHCA routinely conducts surveys of assisted living communities across Florida to help ensure that residents are receiving quality healthcare in a safe environment.

During the re-licensure survey, AHCA field officers reviewed numerous Grand Oaks of Jensen Beach policies and procedures, medical records and staffing reports, among other documents. They also closely observed employees performing their assigned duties. Additionally, AHCA field officers interviewed residents, residents’ family members, and staff members. After the inspection was completed, AHCA determined there were no issues or violations—Grand Oaks of Jensen Beach had earned a perfect survey score!
This deficiency-free survey—as monumental as it is—isn’t the first perfect AHCA survey that Grand Oaks of Jensen Beach has received, earning them an impressive track record of healthcare and operations excellence.
